fish 1. In accordance with the Law of Ukraine "On Personal Data Protection," I, as a personal data subject, voluntarily, consciously, and unequivocally give consent to the Ministry of Digital Transformation of Ukraine, as the personal data controller, to:

1.1. process my personal data (Art. 2 of the Law of Ukraine "On Personal Data Protection") on the website "Rossery.com" Personal Data Processing Notice;

1.2. use my personal data as defined in clause 4 of the Personal Data Processing Notice, which includes any actions by the personal data controller regarding the processing of such data, actions to protect them, as well as actions regarding granting partial or full rights to process personal data to other subjects involved in personal data relations (Art. 10 of the Law of Ukraine "On Personal Data Protection");

1.3. disseminate my personal data to the extent specified in clause 4 of the Personal Data Processing Notice, which includes actions by the personal data controller regarding the transfer of information about an individual (Art. 14 of the Law of Ukraine "On Personal Data Protection");

1.4. provide third parties access to my personal data to the extent specified in clause 4 of the Personal Data Processing Notice (Art. 16 of the Law of Ukraine "On Personal Data Protection").

  1. I hereby confirm that I have been duly informed about the purpose of processing my personal data according to the Personal Data Processing Notice.

  2. According to this Agreement, personal data may be transferred to/from other electronic information resources solely for the purpose of their processing (collection), as defined in clause 5 of the Personal Data Processing Notice, without additional notification of the personal data subject. Personal data may be transferred to third parties as specified in clause 6 of the Personal Data Processing Notice.

  3. This Agreement does not apply to personal data processing actions carried out by other data controllers and processors to whom the Ministry of Digital Transformation of Ukraine transfers personal data to achieve the purpose of processing (collection) specified in the Personal Data Processing Notice.

  4. I hereby confirm that I am familiar with the content of the Personal Data Processing Notice.

  5. The purpose and conditions of processing my personal data are clear to me, and I consent to them.

  6. This Consent is provided by me for the period necessary to achieve the purpose of processing (collecting) personal data, as specified in clause 5 of the Personal Data Processing Notice.

  7. This Agreement is effective from the moment it is concluded.

In accordance with clauses 1 and 2 of part two of Article 8, part two of Article 12 of the Law of Ukraine "On Personal Data Protection," the Ministry of Digital Transformation of Ukraine informs the owner, processor, location, composition, and purpose of the collection of personal data processed by the software of the website "Rossery.com," third parties to whom such personal data are transferred, and the rights of the personal data subject.

  1. The personal data controller processed by the software of the website "Rossery.com" (hereinafter – the processed personal data) is the owner of the website "Rossery.com" – the Ministry of Digital Transformation of Ukraine (legal entity identification code in the Unified State Register of Enterprises and Organizations of Ukraine – 41815683, location: 01001, Kyiv, Mechnikov St., 3, contact phone: +38 093 900 65 35).

  2. The personal data controller of the processed personal data is the owner of the website "Rossery.com" – the Ministry of Digital Transformation of Ukraine (legal entity identification code in the Unified State Register of Enterprises and Organizations of Ukraine – 41815683; location: 01001, Kyiv, Mechnikov St., 3, contact phone: +38 093 900 65 35).
